The role of the Case Manager is to facilitate optimum patient outcomes through a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individuals health needs using communication and available resources to promote quality and cost-effective outcomes through a full continuum of care.

In this role, the selected candidate will perform duties related to the day-to-day operations of the High Risk Case Management functions to include working with members identified as high risk to identify needs and goals to achieve empowerment and improved quality of life.
In this role, you will participate in the collaborative process of assessment, planning, facilitation and advocacy for options and services to meet the beneficiarys health needs through communication and available resources to promote quality cost - effective outcomes.
In this role, the selected candidate will maintain case records detailing service needs and activities/arrangements for their fulfillment, according to data-management procedures established by program management and the funding sources.
In this role, the selected candidate will coordinate the overall interdisciplinary plan of care for a patient, from admission to discharge; acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.
