In this role, you will assess and evaluate patient"s status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and making necessary revisions to the plan.
In this role, the selected candidate will work telephonically with patients identified as high risk, for both behavioral and physical health issues, and their providers to identify needs, set goals and implement action steps towards achieving goals.
In this role, the selected candidate will provide admission, case management, and follow-up skilled nursing visits for hospice patients; administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
