In this role, you will perform telephone triage using best practice guidelines and documentation to ensure patients receive the right level of care in the right setting at the right time by the right caregiver using the right equipment and supplies.
In this role, you will provide safe and effective training and oversight of the delivery of home dialysis therapy to the patient in compliance with standards outlined in the Home Peritoneal Dialysis Policy and Procedure Manual, or/Home Hemodialysis Policy and Procedure Manual , as well as regulations set forth by the Corporation, state and federal agencies.
In this role, you will utilize the nursing process to ensure appropriate patient care is provided in a cost effective, quality driven, and patient focused manner. Collects pertinent social, psychological, developmental and spiritual data from the patient, significant others and other health care team members.
In this role, the selected candidate will maintain appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone.
In this position, you will provide appropriate teaching and information to patients and families including information on Pain Management, Nutrition, Skin Integrity, Fall prevention, Diabetes, Disease Process, Smoking Cessation, and Discharge instructions.
