The SOCIAL WORKER plans, organizes, develops, and directs the overall operation of the Social Services department to ensure all medically-related emotional and social needs of patients are met in accordance with all applicable laws, regulations.
In this role, you will assess the psychosocial status of patients and families/caregivers related to the patients terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patients resources and right to self-determination.
In this role, the successful candidate will formulate, develop and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work diagnosis, treatments and interventions, including crisis intervention, brief individual, marital and family therapies, and patient, family and caregiver groups.
In this role, you will interview patients and relatives to obtain social history relevant to medical problems and planning. Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care.
As a Social Worker II (SW II), the individual comprehensively provides clinical social work and complex discharge planning to patients and their families of a targeted patient population on a designated unit(s).
