In this role, you will provide social work services to patients and families in the acute care setting to meet their medically related social and emotional needs as they impinge on their medical condition, treatment, recovery, and safe transition from one care environment to another using an interdisciplinary approach and coordination of timely discharge planning.
In this role, you will be responsible for providing case and group work services during the hospital stay. The social worker acts as an advocate on behalf of the patients to ensure maximum access to quality healthcare.
In this role, the selected candidate will provide ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
The selected candidate will participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
