In this role, you will be responsible for The Integrated Health Care Clinician provides leadership, motivation and support to staff, manages the referrals, admission and discharge processes for clients, and ensures the integrity of the program model.
In this role, you will deliver direct services and coordination of services to clients, and may provide other authorized therapeutic modalities as relevant participates in interdisciplinary treatment planningand ensures that any treatments delivered are within the scope of his or her training, experience, and general level of competence.
Identify and evaluate referred individuals served risk issues and develop appropriate critical need interventions and transition plans; ensure timely completion, with individuals served and other members of the Trans-Disciplinary Team, the Comprehensive Assessment and other clinical assessments as needed.
In this role, you will have access to an extensive virtual training library, which assists in meeting CEU licensure requirements. The candidate must possess good organizational skills, as well as effective verbal, written and computer skills.
Overseeing and coordinating overall patient case management with applicable state and local social services agencies, legal referral sources and resources, school-based stakeholders, and community-based providers of services, as needed.
Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care.
