In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
The Hospice RN Case Manager is a skilled professional member of the Interdisciplinary Team, responsible for implementing the nursing component of the plan of care and coordination of other clinical and support services. The position also provides direction and supervision to hospice aides and serves as the primary liaison with patients physicians and ancillary health providers.
The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit design and/or acute health needs through communication and available resources to promote optimal, cost-effective outcomes.
In this role, you will be responsible for effectively managing or caring for a caseload of patients from admission to transmission into the community, involving interdisciplinary support and referrals when appropriate.
In this role, you will work to achieve daily clinical, quality and cost outcomes by providing well-coordinated experiences for patients/families through the synchronization of care activities of multiple disciplines and negotiation with third party payers.
This position is responsible for having a thorough understanding of patient treatment plans through participative discussions with the care plan (interdisciplinary) team, identifying and referring requests for services to the Medical Director when guidelines are not met and reviewing inpatient and outpatient services requiring MCO approval.
In this role, you will plan, directs, and supports both clinical and administrative staff to ensure that they provide mental health services to an adult mental health population in the community on a 24/7 hours basis. Assures departmental goals and/or objectives are accomplished within prescribed time frame by performing the job responsibilities personally or through subordinate staff.
