In this role, you will build meaningful relationships with a specified number of seniors and their families as you provide assistance with activities of daily living, attend to individual care needs and get to know their unique preferences and personalities.
The Care Coordinator in collaboration with the multi-disciplinary team evaluates the responses to the individualized plan of care gathering data from the EMR, patient re-assessments, reports, and support systems.
In this role, you will help to facilitate the daily activities of the patient oriented delivery system along with helping to assure ongoing quality, productivity and efficiency in the daily operations of the population health coaches.
The care coordinator, in collaboration with the multi-disciplinary team, performs a focused assessment to evaluate patients and their support systems for real and potential health and safety issues, educational needs, and resource availability upon initial interaction, throughout care and/or treatment, and at times of transition.
