In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.

In this role, you will provide technical support for Vision Center of Excellence tasks aligned to the Education, Training, Simulation, and Readiness to promote the prevention of eye injuries and enhance readiness and ocular trauma preparedness across the DoD, VA, and civilian communities.
