The primary purpose of this position is to assist customers with their health care needs by filling prescriptions and providing excellent customer service and assisting with supervising Pharmacy associates.
The Pharmacist will respond to questions from prescribers, nurses, pharmacy technicians , support staff and patients. The Pharmcist will provide friendly, appropriate counseling to a diverse patient population.

The clinical pharmacist is responsible for providing medication therapy management services in collaboration with the interdisciplinary care team to assure that drug therapy is safe and effective for kidney transplantation and cross coverage for thoracic services.
The pharmacist will utilize integrated and routine communication with all health care professionals and the patients concerning medical care and quality of life with the emphasis placed on the monitoring and documentation of specific pharma-cotherapy outcomes.
In this role, you will be responsible for utilizes risk stratification tool to identify targeted high risk patient population dependent on age, hospitalizations, ED visits, comorbid diagnoses (e.g. COPD, CHF, CKD), polypharmacy and behavioral health diagnosis.
