The RN Care Manager leads the Primary Care Teams and is responsible for the care management of the patient's assigned to those teams and for providing the physical care that requires the expertise of a Registered Nurse.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients.
Collaborates with medical staff to monitor patient outcomes; set clinical quality priorities, recommend and implement professional improvements and initiatives and implement practice-based safety initiatives.
The phn performs the personal health management process, that is assesses the participant, works with the participant, family and physician to identify problems, establish goals and develop plans of care, coordinates services, educates participants, empowers participants to independently self-manage and to make knowledgeable health care decisions.
