Collaborate and integrate with onsite clinic to educate members on NCRs available programs and resources, and provide both Case management and Lifestyle coaching to NCR members that are referred for conditions that could benefit from these programs.
In this role, the selected candidate functions as the liaison between the employee/patient, supervisor, medical/rehabilitation treatment team and benefits administrator, facilities coordination of disability management and return to work services to employees/patients through effective management of available resources.
In this role, you will work with foster children, their family/guardian, and foster parents to ensure the provision of comprehensive case management services. Must be able to conduct intake placements, advocate for clients in court, conduct home visits, assess needs of children and families, transport clients as required, and complete required paperwork. Must be able to manage crisis situations as they arise.
In this role, you will use clinical proficiency, claims knowledge / analysis and comprehensive knowledge of health care continuum to assess, plan, implement, coordinate, monitor and evaluate medical necessity, options and services required to support members in managing their health, chronic illness or acute illness.
Under the direction of the therapeutic foster care supervisor, the Case Manager will work with foster children, their family/guardian, and foster parents to ensure the provision of comprehensive case management services.
Assumes full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered.
In this role, you will coordinate the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.
