The selected candidate will implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
In this role, you will provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes.
In this role, you will participate in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
In this role, the selected candidate will be responsible for the provision of medical social work services to patients receiving care in the hospital setting; assesses the social, psychological, cultural, environmental, and financial situations, as well as, disposition needs, for each referred patient.
