The Clinical Nurse, Resource Team, provides comprehensive phone triage nursing care through skillful assessment, diagnosis, outcomes identification, planning, implementation, and evaluation in accordance with facility or department policies procedures, and/or protocols under the supervision of the nursing leader.

In this role, you will prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.
Nurses are responsible for providing IV infusion nursing care in the home or ambulatory setting, performing in-hospital discharge assessments and occasional IV hook-ups, teaching patients about their home infusion therapy, and providing ongoing monitoring of their treatment.

In this role, the successful candidate will document all pertinent information regarding nursing care, care plans and observations; implement and coordinate the delivery of care in collaboration with physician and resource health care personnel.
In this role, you will provide daily care coordination, case management, coaching, consultation and intervention to patients with one or more chronic diseases in order to promote self management and preventive health care to drive better patient outcomes in the outpatient setting.
In this role, the selected candidate will provide admission, case management, and follow-up skilled nursing visits for home health patients; administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
