The Social Worker provides social work services, complex discharge planning, and care coordination to elective surgery patients and their families within the context of the Surgical Navigation Center's interdisciplinary care model.
In this role, you will provide information, education to patients and their social support on community resources and options for post hospital care appropriate to the age of the patients served, in collaboration with the Clinical Care Management team. Make referrals to community agencies as needed.
In this role, you will develop, maintain and implement record review tools for each of the Child Welfare programs ensuring that review elements are consistent with state, federal, and other monitoring requirements.
The selected candidate will implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
