The care manager will be responsible for providing case management to beacon members in the commonwealth coordinated care program (medicare & medicaid dual eligible) that experience serious mental illness on an ongoing basis.
This position is responsible for day-to-day operations for the Hospitals Transfer Center and Bed Placement functions, providing management direction to staff as well as building and maintaining productive relationships with referring/receiving providers and other stakeholders.
Assesses, plans, coordinates and evaluates the clinical and operational functions of the nursing unit to ensure quality cost effective healthcare is provided to the residents. Accepts responsibility and accountability for total unit functioning. Performs nursing care as needed.
In this role, you will develop communicative relationships with them while managing scheduling and maintain effective fiscal management by monitoring metrics (gross margin, overtime, unfilled hours, etc.).
In this role, you will works with patients to increase their knowledge level on health care conditions and provides information to improve their health. Also works with transition care clinic with referrals as needed.
Works collaboratively and maintains active communication with the multidisciplinary care team including providers, pharmacists, social workers, behavioral health specialists, and nurses to achieve timely, appropriate patient management.
