In this role, you will provide input to care plan development and inform the Company location clinical staff (e.g. Intake Specialist, Director Clinical Management, and/or Manager Clinical Practice) of patients acceptance, and ensures that patients needs are appropriately matched.
In this role, the selected candidate will be responsible for members gaining access to needed services through coordination and integration of medical and long term care services for the purpose of orientation, care plan development, assessment, and care coordination.
This position will develop, implement and manage a comprehensive patient safety program that identifies and addresses the prevention of errors and evaluation of risk events to determine opportunities to improve patient safety and care delivery.
In this role, you will conduct screening and risk assessment interventions per program guidelines. Provide outreach and preventative care to members seeking services in the least restrictive environment.
In this role, the selected candidate will educate providers, members and their families on behavioral health issues including symptoms, relapse prevention, stress reduction, lab work, and healthy lifestyle choices.
In this role, the selected candidate will communicate significant findings, problems and changes to Manager, Clinical Services and physician, and documents all findings, communications, and appropriate interventions.
