The Nurse Practitioner is an independent health care provider who, working collaboratively within a multi-disciplinary heatlh team, is responsible for patient evaluation and care through the performance of pertinent history and physical examination, assessment of needs and delivery of appropriate treatment.
In this role, the candidate will deliver patient care in mobile environment as part of inter-professional collaborative practice including physicians, paramedics, nurses, podiatrists, pharmacists, health workers and others.
The Nurse Practitioner (NP) 2 works as part of the Clinical Operations Team, providing clinical assessment and care to assigned patients in their homes and in skilled nursing facilities; identifying quality improvement activities and planning individualized health care consistent with Matrix Medical Network policy protocols.
In this role, you will work collaboratively with primary care physicians to manage groups of patients in the primary care office setting to improve access, improve patient outcomes and promote evidence-based medicine.
In this role, the selected candidate will conduct comprehensive or episodic health assessment of adolescent, adult and geriatric patients with real or potential acute and/or chronic health problems which may include the psychological, cognitive and personal assets and limitations of the patient/family, with an emphasis on wellness.
