The Case Management Department provides our patients and clinical staff with support to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post-hospital needs.
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
In this role, you will excellent verbal and written communication skills. Knowledge of acute psychiatric disorders and community resources desirable. Must have flexibility to cover other functions as needed.
In this role, you will be responsible for the patient/family unit, defining social service goals for alleviating these problems, and helping to provide both the counseling and casework needed to meet these goals.
