This role will utilize multiple disciplines as CM to focus on various different patient populations.The Case Manager
acts as an advocate for members and their families linking them to other appropriate disciplines on the care team.
The Workers Compensation Field Case Manager is responsible for assessing and analyzing injured employees to evaluate their medical and vocational needs. The Workers' Comp Field Case Manager must facilitate the patients appropriate and timely return to work, acting as a liaison between the patient/family, employer, providers, insurance carrier, and other healthcare personnel.
This individual is responsible for the medical case management of work related injuries which includes assessment, planning, coordination, implementation and evaluation of. injured/disabled individuals.
In this role, you will be responsible for performing care management within the scope of license for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans.
Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care on a daily basis.
In this role, the selected candidate will develop the patient's Plan of care in collaboration with other professionals and implements treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being.
