Provides psychiatric social work services at time of admission through discharge. Coordinates discharge planning for patients admitted to Adult or Geropsychiatry consistent with hospital and departmental policies, procedures, standards, and quality assurance measures.
In this role, you will implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
Reviewed patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients representative to ensure patients understanding of the rights and expectations of them.
Provides assessment, diagnosis, treatment and crisis intervention as needed to members who present themselves for psychiatric evaluation. Collaborates with treating physician, psychiatric and allied health professional team to plan and direct each individual member's treatment program.
Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
