In this role, the selected candidate will provide case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patient care.
In this role, you will performs admission certification screening for patients in accordance with utilization plan using isd criteria, and performs concurrent stay reviews at least every 72 hours for medical necessity using isd criteria. Maintains documentation on discharge planning notes and charts on patients record.
The Case Manager is responsible for providing case management and support activities for New Directions members who have been: discharged from inpatient or residential treatment settings, need assistance during a signification treatment transitions, or have co-occurring medical and behavioral health conditions and who meet the NDBH eligibility criteria for integrated case management.
The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
