In this role, you will manage one-time task assignment or full field case management referral to meet with attending physician, injured worker and/or employer to clarify medical and/or vocational claim related information which could not be obtained through correspondence or telephonic attempts by claims team.
In this role, you will performs admission certification screening for patients in accordance with utilization plan using isd criteria, and performs concurrent stay reviews at least every 72 hours for medical necessity using isd criteria. Maintains documentation on discharge planning notes and charts on patients record.
This individual is responsible for the medical case management of work related injuries which includes assessment, planning, coordination, implementation and evaluation of. injured/disabled individuals.
In this role, you will coordinate the integration of social services / case management functions into the patient care, discharge planning processes in collaboration with other hospital departments, external service organizations, agencies and healthcare facilities.
