In this role, you will collaborates with clients, families and providers by assessing patient needs, facilitating and implementing clinical care, planning, education, monitoring and evaluating specific services and advocating health care needs on an individual basis either by phone and/or by onsite visits.
In this role, the selected candidate will perform duties related to the day-to-day operations of the High Risk Case Management functions to include working with members identified as high risk to identify needs and goals to achieve empowerment and improved quality of life.
In this role, you must have have a current RN license in the State of Ohio, 1-2 years home care or community nursing experience, demonstrated excellence in nursing assessment, home care case management, communication and problem-solving skills.
The Case Manager possesses strong clinical skills and a knowledge base in a specialized area, and utilizes leadership skills in coordinating and/or providing home care services for a group of patients within that specialty area.
In this role, you will assesses and evaluate patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and making necessary revisions to the plan.
In this role, you will complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs. He / She will provide direct nursing care to residents, in compliance with the Nurse Practice Act, and federal, state and local laws.
As a Registered Nurse Case Manager, you will partner with our team from Health Services to utilize a collaborative process of assessment, planning, and advocacy to meet a members benefit plan and/or health needs.
