Perform administrative responsibilities: i.e. update patient charts, billing codes and completing of required documents from outside agencies: i.e. schools, employers, New York State Department of Social Services, etc.
The Nurse Practitioner, in collaboration with a physician, provides for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.
The Certified Registered Nurse Practitioner will provide clinical assessment and intervention for consumer care provision in a manner consistent with the philosophy, mission, and vision of the organization.
Performs, under the responsibility and supervision of the surgeon, diagnostic and therapeutic tasks to allow the physician to extend services through the more effective use of knowledge in the sphere of decision making required to establish a diagnosis and to plan therapy.
Will work in collaboration with other providers and disciplines as well as community service agencies such as the Upstate NY chapter of the National Multiple Sclerosis Society to provide coordinated care.
The Nurse Practitioner (NP) works as part of the Clinical Operations Team, providing clinical assessment and care to assigned patients in their homes and in skilled nursing facilities; identifying quality improvement activities and planning individualized health care consistent with Matrix Medical Network policy protocols.
Assesses & evaluates patients, orders appropriate laboratory testing and diagnostic studies, arranges for referrals, testing, consultation, and therapeutic services, and maintains outstanding relationships with families, referring physicians and all members of the health care team.
