The primary responsibility of the Case Manager is to identify, screen, track, monitor and coordinate the care of patients with multiple co-morbidities and/or psychosocial needs and develop a case management plan of care.
The Case Manager is responsible for facilitating appropriate health care services throughout the continuum of care. Uses collaborative and systematic processes to assess, plan and coordinate the implementation of services that meet the individual's health needs.
Collaborates effectively with integrated care team to establish an individualized plan of care for members. The integrated care team includes physicians, case managers, referral coordinators, pharmacists, social workers, and other disease educators.
This position integrates care coordination, utilization management, and discharge planning. this position has overall responsibility for overseeing the clinical plan of care to conform to evidence-based practice and regulatory requirements.
The Case Manager is responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file found on Southwest Key Programs (SWK) designated web-based program with supporting documentation maintained in physical files.
