Provide coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices; help member actively and knowledgably participate with their provider in healthcare decision-making.
Helps provide access to the right care, at the right time, in the right order, in the right setting with the right documentation. Responsible for assisting high-risk patients in achieving maximum wellness through a collaborative, interdisciplinary process involving the patient, family and/or care-givers as well as providers and other clinical support staff. This process includes assessment, planning, facilitation and advocacy.
Responsible for providing day-to-day operations, clinical and administrative leadership to the team members, charged with the provision of comprehensive, patient-centered, quality health care for pediatric patients.
This position will contribute to the success and effectiveness of the SDC department by collaborating with the SDC Leadership Team (LT) in leading the whole of staff training, development and care together as a primary responsibility, with management and oversight of crisis care as a secondary responsibility
The ideal candidate will direct the day-to-day functions of the facility in accordance with current federal, state, and local standards, guidelines, and regulations that govern nursing facilities to assure that the highest degree of quality care can be provided residents at all times.
This position provides managerial and operational oversight of clinical departments; develop implements, and supports changes in process that support quality improvement, excellent patient care, and financial responsibility.
