In this role, the selected candidate will coordinate total nursing care for patients, participates in patient and family teaching, and provides leadership by working cooperatively with ancillary nursing and other patient team personnel in maintaining standards for professional nursing practice in the clinical setting.
In this role, you will work requires a professional level of knowledge of nursing practice and theory at a level that generally can be acquired by completion and graduation from an accredited School of Nursing.
In this role, you will provide admission, case management, and follow-up skilled nursing visits for home health patients; administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
In this role, you will provide professional nursing care for physician practice patients following established standards and practices. Greets patients, takes initial information and prepares them for physical examination. Performs general nursing care as prescribed by the physician including administering medications (injections, IV therapy, distributing sample medications), performing therapeutic treatments, patient assessment and education.
