The Clinical Liaison is responsible for assessing the needs of assigned referrals. Based on needs assessments, develops solutions that meet identified needs, which result in inpatient and/or outpatient referrals.
As a Clinical Nurse Liaison you will serve as an expert consultant to hospital staff, and a clinical educator and advocate for patients, with the goals of raising awareness about the benefits of home infusion and optimizing the transition of care for patients referred to BioScrip.
In this position, you will be working collaboratively with business development team members including the Admissions Manager and admission staff, as well as nursing and other internal and external staff to facilitate the referral conversion.
In this role, you will provide patient and caregiver training on therapy, monitoring, and side effects; demonstrate procedure for administering therapy in the home, with a return demonstration from the patient / caregiver.
This position will be a critical member of the sales and admissions team, and will develop relationships with key hospital personnel such as administrative management, discharge planners, local physicians, case managers and social workers to help increase referrals and ensure a dignified and smooth discharge experience for patients/ residents transitioning to their communities.
