In this role, the selected candidate will provide admission, case management, and follow-up skilled nursing visits for hospice patients; administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
Performs a nursing assessment on all patients. The data collected forms the basis of an individualized plan of care. Identifies teaching needs and goals related to the diagnosis of each patient from the nursing admission assessment and by collaborating with patient/family and the Interdisciplinary team.
This position is responsible for supervising staff involved in case management activities and support for Case Management programs, developing and implementing managed care policies and procedures related to Case Management, and monitor and assist department in achieving goals.
Provides education on hiv/aids and other co-morbidities including treatment adherence to medication regimens, including provider appointments, prevention, harm reduction, sexual health and preventative health measures, (colorectal screening, pap smears, psa screening, etc.)
The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings.
The Case Manager identifies, evaluates, and provides management of services for patients with complex, catastrophic, long term illness or injury, targeted diseases, as well as those patients receiving out of network services.
