Performs and supervises all activities necessary to process payroll, including maintaining related records, filling tax reports and voluntary deduction reports, processing involuntary deductions such as levies and garnishments, preparing accounting transactions and documents, documenting and updating procedures, and preparing special reports for management.
The Home Health Case Manager is a RN possessing special knowledge of care practices and regulations required for provision of comprehensive care to patients in their homes, managing the care plan for a group of assigned patients.
Works with physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
This role provides twenty-four (24) hour accountability for the delivery of quality patient care that promotes the safety and well-being of all patients on assigned units. Responsible for planning, prioritizing, organizing, directing and meeting the daily needs and demands of patients and staff on assigned nursing units.
Responsible forinitial utilization screen and ongoing utilization review throughdischarge. Identifies discharge andtransition needs and is ultimately responsible for the coordination ofdischarge planning activities.
In this role, the selected candidate will be responsible for working with the project construction contractor to monitor construction schedule; insuring that all construction complies with engineering drawings and specifications and company quality standards.
Monitor referrals made to community based organizations, medical care and other services to support the members overall care management plan; enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems.
