In this role, the selected candidate will be responsible for creating an environment that leads to a positive patient care experience by evaluating, planning and implementing effective plans of care and communicating with both patients and their families.
As a Home Health Registered Nurse, you will provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision.
In this role, the selected candidate will develop and implement individual plans of care in collaboration with other members of the healthcare team in accordance with the established guidelines and standards of nursing care.
This position is responsible for delivering nursing care to patients requiring care, collecting patient data, making observations and reporting pertinent information to the care of patients. Will also direct and supervise care given by other nursing staff members as needed.
The Registered Nurse works under direct supervision using the state-specific Nurse Practice Act, Practice Guidelines and nursing judgment.
