As a Telephonic Social Worker, you will be responsible for providing social work services to patients and families. You will function as a member of the departmental team which includes educating the patient, family and members of the healthcare team regarding benefits, community resources, referrals for counseling and other pertinent information.
In this role, you will provide the professional Social Work component of the inpatient Care Management model to ensure that department services support quality patient care and safety, communication, education and financial components while coordinating patient throughput.
This position is responsible for the development and delivery of social work service, to promote access to funding mechanisms for patients in need of care, and to promote interdisciplinary collaboration for the support of emotional and social well-being of patients and families.
