Collaborates with patients, caregivers, internal/external healthcare providers, agencies and payers to plan and execute a safe discharge; re-evaluate and revise discharge plan as patient clinical condition merits.
In this role, you will perform duties to conduct and manage the day-to-day operations of the case management functions communicating with departmental and plan administrative staff to facilitate daily department functions.
In this role, the selected candidate will direct the day-to-day functions of the facility in accordance with current federal, state, and local standards, guidelines and regulations that govern long term care facilities, as well as all company policies to assure that the highest degree of quality care can be provided to the residents at all times.
In this role, you will be responsible for appropriate application of medical policy and criteria. Communicating and executing corporate initiatives. Management and control of departmental expenditures defined by cost center allocation.
In this role, you will collaborates with clients, families and providers by assessing patient needs, facilitating clinical care, planning, education, monitoring and evaluating of specific services either by phone and/or by onsite visits.
