In this role, the selected candidate will be responsible to return them to their optimal level of function. In addition, acts as needed as member of the hospice interdisciplinary team providing in-home assessment/evaluation, patient/family/caregiver education and inservice training for both staff and volunteers.
In this role, you will participate as a member of the health care team, working with professional and non-professional staff and associated professionals in providing care which provides optimum home care and continuity of care to referred patients, addressing the age of the patients served.
The Physical Therapist performs initial evaluations for newly referred patients to determine the primary and secondary problems involved, to collect baseline data regarding findings of tests and measurements and other appropriate factors, and to determine the appropriateness of the prescribed care.
The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records.
