In this role, the selected candidate will provide drug information and support for clinical pharmacy services, including clinical monitoring, clinical consults, patient care conferences, multidisciplinary patient rounding, oncology services, kinetics dosing and TPN monitoring.
In this role, you will review and assess medication orders to identify, prevent and resolve potential and actual drug related problems. The selected candidate will communicate with physicians and other clinicians as necessary to facilitate care.
This position is responsible for performing pharmacy program quality improvement activities including but not limited to delivering comprehensive Medication Therapy Management (MTM) services to physicians and members via outbound and inbound communications.
The Clinical Client Operations Pharmacist will work within the vendor transition area on new client implementations related to prior authorizations; work with various clinical forms related to client formulary to ensure previous vendor member overrides are properly compared to all clinical programs and formularies.
In this role, you will assume overall responsibility for the acquisition, storage and dispensing of medication; Interprets and verifies physician orders; Enters medication orders into the computer system; Provides information and consultation to healthcare professionals and patients.
In this role, you will conduct drug utilization review studies. Consults with medical practitioners on prescription orders, patient reactions, and errors. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks.
