The Pharmacist will be responsible for practicing Pharmacy as a knowledge based clinical profession, and integrating patient oriented services with traditional distributive and drug control activities.
Responsible for the coordination of specialty pharmacy services in assigned area; provide consultative/clinical services including antibiotic dosing of vancomycin and aminoglycosides, initial Coumadin dosing, renal dosing and IVto-PO conversions.
Provides pharmaceutical services including drug information to nurses, physicians, and other health care providers. Serves all patients to include, but not limited to, neonates, pediatrics, adolescents, adults and geriatrics.
Work directly, either individually or in large group settings, with prescribers to identify and address prescription utilization trends and issues of clinical performance and effect change in medication quality measures.
