The Case Manager coordinates the care and service of selected patient populations across the continuum. He/she works collaboratively with patient, family, physician and other members of the health care team to achieve the highest quality clinical outcomes with the most cost effective use of available resources.
Responsible for providing and supervising primary care to approximately 25-35 patients in their home setting. This includes age appropriate interventions that meet physical, emotional, developmental,psychosocial and educational needs.
In this role, the selected candidate will assess and analyzes injured workers medical and vocational status; develop progressive plan of care to facilitate medical recovery and progressive return to work.
Collaborate and integrate with onsite clinic to educate members on NCRs available programs and resources, and provide both Case management and Lifestyle coaching to NCR members that are referred for conditions that could benefit from these programs.
The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
