Responsible for the daily management of an assigned patient caseload, ensuring timely care and documentation to meet the patent needs/services as prescribed in the established plan of care and per MD orders.
The RN Coach will function in a telephonic virtual care center and is part of the Care Connections Team which collaborates with physicians, mid-level providers, staff, and other health care professionals to provide coordination of care across the health care continuum.
Provides safe and effective training and oversight of the delivery of home dialysis therapy to the patient in compliance with standards outlined in the FMS Home Peritoneal Dialysis Policy and Procedure Manual, or/Home Hemodialysis Policy and Procedure Manual.

The nurse uses clinical skills to coordinate, document and communicate all aspects of the utilization management program. Utilizes clinical experience and skills in a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options to facilitate appropriate healthcare services/benefits for members.
Additionally, this position collaborates with quality, patient safety, teammate health and patient care team to provide coordinated leadership and support to achieve optimal infection prevention and control outcomes.
Responds appropriately to emergency situations (Cone Health Codes) and demonstrates knowledge of how to respond. Always uses appropriate safety devices and techniques (examples: using safe lift equipment and needle safe devices; pushing carts; correctly lifting copy paper).
