In this role, you will be responsible for assessing and analyzing an injured employee to evaluate the medical needs required to facilitate the patients appropriate and timely return to work. Acts as a liaison with patient/family, employer, provider(s), insurance companies, and healthcare personnel.
The Case Manager is responsible for telephonically assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs, overall wellness and/or appropriate and timely return to work.
In this role, you will use clinical proficiency, claims knowledge / analysis and comprehensive knowledge of health care continuum to assess, plan, implement, coordinate, monitor and evaluate medical necessity, options and services required to support members in managing their health, chronic illness or acute illness.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Provide admission, case management, and follow-up skilled nursing visits for home health patients.Administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
