In this role, you will be responsible for the coordination and delivery of care at the appropriate level and implementation of discharge planning activities for patients within the acute care setting by providing a systematic, collaborative approach in the management of patients that assures quality and cost effective patient care, appropriate documentation to support the level of care and continuity of care after discharge.
In this role, you will work collaboratively and maintains active communication with the multidisciplinary care team including providers, pharmacists, social workers, behavioral health specialists, and nurses to achieve timely, appropriate patient management.
In this position, you will be responsible for the performance of standardized comprehensive needs assessment, determination of available benefits and resources; and development and implementation of a plan of care for assigned patients that includes performance goals, monitoring, follow-up and outreach activities.
In this role, you will perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care.
In this role, you will gather and analyze data and apply methodologies. Draw preliminary conclusions and project application(s). Identify cross-application of data or analysis, directing efforts of other associates, as needed.
