As a Home Health Registered Nurse, you will provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision.
Looking for a Registered Nurse -Emergency Department Nights whose compassion, clinical expertise, and focus on patient care are top priority. As an RN in our Emergency Department you will conduct patient assessments, develop and implement nursing care plans to foster quality outcomes.
In this role, you will be responsible for delivering nursing care to patients requiring care, collecting patient data, making observations and reporting pertinent information to the care of patients. Will also direct and supervise care given by other nursing staff members as needed.
In this role, the selected candidate will provide admission, case management, and follow-up skilled nursing visits for home health patients; administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
The Registered Nurse is responsible for the nursing care of patients according to the physician's orders. The RN is responsible for assessing, planning, implementing, and evaluating total patient care and nursing care plans for each patient.
The Registered Nurse works under direct supervision using the state-specific Nurse Practice Act, Practice Guidelines and nursing judgment. Responsible for delivering nursing care to patients requiring care, collecting patient data, making observations and reporting pertinent information to the care of patients.
In this role, you will be responsible for coordination and Communication with Interdisciplinary team members for Physician, Social Worker, Spiritual Care Coordinators, Bereavement Coordinators, Volunteer and Dietician services, as needed.
