The Registered Nurse (RN) delivers efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction. He/she operates within the scope of practice defined by the State Nurse Practice Act and delegates aspects of patient care to LPNs and Caregivers/Aides, consistent with their scope of practice.
In this role, you will provide admission, case management, and follow-up skilled nursing visits for home health patients; administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
Responsible for providing IV nursing care in the home or ambulatory setting, performing in-hospital discharge assessments and occasional IV hook-ups, teaching patients about their home infusion therapy, and providing ongoing monitoring of their treatment.
