In this role, the selected candidate will be responsible for ensuring the patients medical, psychosocial, resource and spiritual needs are identified and a plan is developed and implemented to address identified needs.
The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
In this role, the selected candidate will provides comprehensive care coordination for patients as assigned. This position assesses the patients plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patients health care needs.
This position plans and provides clinical support to patients and staff. Demonstrates clinical expertise of the assigned area. Provides leadership in clinical, financial, and personnel management within the department.
