Utilizes clinical experience and skills in a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options to facilitate appropriate healthcare services/benefits for members.

The RN MDS COORDINATOR coordinates and assists with completion and submission of accurate and timely interdisciplinary MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, regulations, and Life Care standards.
Provides high quality professional skilled nursing services to the ill, injured, convalescent, terminally ill and/or handicapped patients in such settings as private homes, nursing homes and similar institutions.
Supervise nursing service activities in patient unit for the duration of one shift and/or in the absence of the manager. Responsible for ensuring patient care is delivered in accordance with quality standards on the shift.
