Performs evaluations, treatment, planning and implementation, and discharge planning for all patients; manages all assigned patients, paperwork that is associated, and aids in all department functions that are assigned by supervisor.
Maintains contact/communication with other personnel involved in the patients care to promote coordinated, efficient care; instructs patients family/caregiver and other Organization health care personnel in patients treatment regime when indicated and appropriate.
Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives. Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patients level of function.
Responsible for a continuum of services providing care to patients with neuromuscular, orthopedic and other physically-related disorders requiring mobility training, strengthening, modalities, specialized manual therapy and neuromuscular re-education.
Evaluate and treat residents and patients with conditions that are mentally, physically, developmentally, or emotionally disabling to improve their ability to perform tasks of daily living and working environments in compliance with federal guidelines and state practice acts.
Completes evaluations, develops written treatment plan and provides ongoing treatments that are designed to improve the patient's functional status in conjunction with the overall treatment objectives for a given patient.
