The Pain Management Rounds Nurse performs clinical care activities and is responsible for the coordination of a specialty pain management service for patients, physicians, and other identified customers.
Performs a complete assessment of assigned patients ongoing physical and emotional status, level of competency, home environment, safety factors and family support available to assist with care and needs of patient.
Train the home dialysis candidate/Home Partner as appropriate on the safe and effective operation and maintenance of all home dialysis equipment and treatment supplies through an organized and formalized Home Dialysis Training Program.
The Nurse must demonstrate proficient skills using Oasis to admit, transfer, re-certify and discharge home health patients in regards to physician orders. Coordinate and supervise LVNs and HHAs in the delivery of patient care while following Medicare regulations, as well as company policies and procedures.
Maintains competency with all emergency operational procedures, and initiates CPR and emergency measures in the event of a cardiac and/or respiratory arrest when the patient is in the dialysis facility.
Provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation. Participate in the department staffing and the appropriate hiring, firing and disciplinary actions.
This individual will assess patient and document findings, administer medications, as well as assess and address patient/family educational needs regarding end-stage renal disease, dialysis therapy, diet and medications.
