The Office Nurse is responsible for ensuring that patient care is coordinated and managed properly. The Nurse will work in collaboration with case management to facilitate discharge planning, gather information about a new referral to home health and obtain Face to Face documentation from physicians to ensure that all requirements are met.

In this role, the selected candidate will provide admission, case management, and follow-up skilled nursing visits for home health patients; administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
