The Nurse must demonstrate proficient skills using Oasis to admit, transfer, re-certify and discharge home health patients in regards to physician orders. Coordinate and supervise LVNs and HHAs in the delivery of patient care while following Medicare regulations, as well as company policies and procedures.
The Office Nurse is responsible for ensuring that patient care is coordinated and managed properly. The Nurse will work in collaboration with case management to facilitate discharge planning, gather information about a new referral to home health and obtain Face to Face documentation from physicians to ensure that all requirements are met.
In this role, the selected candidate will maintain appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone.
In this role, the selected candidate reports adverse patient events at home or in the clinic, as well as equipment and technology related problems to the Clinical Manager/Home Therapy Program Manager, and physician including the correct documentation of such events.
In this role, the selected candidate will provide admission, case management, and follow-up skilled nursing visits for home health patients; administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
In this role, you will be responsible for the delivery of skilled nursing care and supportive services within the scope of the nurse's education and experience. Compliance with Heartland Corporate Standards and local, state, and federal laws, in an institutional setting, within the policies of the institution. Supplements nursing care of clients.
In this role, you will coordinate the plan of care. Conduct patient teaching specific to disease entity and home management. Actively participate in assuring care continuum. Collaborate with other nursing and ancillary services to assure seamless communication and uninterrupted patient care.
In this role, the selected candidate will perform patient assessments, develop patients care plans, evaluate patients care plans effectiveness, modify as appropriate, and document all patient care as appropriate.
