The RDHO will work closely with the leadership team of assigned locations to implement new initiatives and develop, implement and monitor action plans to improve the quality, efficiency and effectiveness of current services.
This position also functions as a staff pharmacist up to 50% of the time as assigned. He or she applies CHW Core Values to the performance of every job duty. These Values include Dignity, Collaboration, Justice, Stewardship, and Excellence.
This position assures that the medication orders are consistent with current medical practice, F.D.A. and drug manufacture recommendations, current literature and/or other information sources. The Pharmacist resolves variations from the above standards with members of the Medical Staff in accordance with established policies and procedures.
The Clinical Pharmacist/Clinical Pharmacy Analyst requires a clinical pharmacy and data analytic background to leverage integrated pharmacy and medical data help identify members who meet prior authorization criteria for select drugs (e.g. criteria based on age, diagnosis, prior use select drugs, etc.) so that the prior authorization can be granted for the member without impacting the member and provider experience at the pharmacy (AKA Smart Edits).
In this role, the selected candidate will provide medication therapy management including examination of member medical records, performing a comprehensive medication review and communicating with members and providers, including outbound calls to members to discuss their medications.
